Case management

Case management services for our clients is provided through Information & Referral (I&R), but located in our Harmony House day shelter. The I&R case manager meets with each individual seeking services from Harmony House. He or she will complete a comprehensive assessment of their needs and determines eligibility for other services we offer. For example, he or she will gather background information as well as their most immediate need. Then, he or she will make the appropriate referrals to have those needs met (i.e. vision, housing, employment).
